  3. Thank you for your interest and support of DCTV.  The following form
  4. should be completed in triplicate and sent to Digital Creations.  After
  5. going through the approval process we will send you a copy of this
  6. agreement along with the most current version of the dctv.library.  Also
  7. if you are interested we have DCTV compatible stickers which are available
  8. upon request at no charge. We require that you send us a copy of your
  9. product when it supports DCTV. This allows us to support your software
  10. with recommendations, and to make sure your product supports ours in a
  11. complementary fashion.  Please read the following and complete as

  29. This form is to be filled out by companies wishing to support Digital
  30. Creation's DCTV display and digitizing device.  Proper filing of this form
  31. with approval from Digital Creations allows the registered companies the
  32. right to distribute the file "dctv.library" in it's most current version
  33. with the product listed below.

  35. . This application only applies to commercial products.  dctv.library may
  36.   not be distributed with public domain or shareware products.
  37.  
  38. . A separate registration is required for each product/program.

  40. . You must provide Digital Creations with a review copy of any product with
  41.   which you intend to distribute dctv.library prior to release.
  42.  
  43. . Renewal is required every two years.
  44.  
  45. . Digital Creations retains the right to revoke distribution rights of the
  46.   dctv.library at any time.

  48. Digital Creations will keep you posted with the most current version of
